Where would you like the event to be advertised?

• The Request Form must be submitted at least 2 weeks prior to the date you would like the information to be posted.
• All pictures and documents must be submitted in a digital format. The preferred format for pictures is a JPG file. The preferred format for documents is a PDF file.
• Additional required materials can be submitted electronically to the following email address (
• The Ministry Leader/Liaison Signature does not have to be included on the form when submitting the request electronically. However, a hardcopy of the form with the signature must be placed in the IT Committee’s mailbox.
• An IT Committee member will contact you if additional information is needed.
